Why Safety, Credentials, and Long-Term Outcomes Matter More Than What’s Going Viral



A few weeks ago, a 57-year-old woman — I’ll call her Lucille (not in the photograph above) — came to see me for a consultation. She found me online through a TikTok account advertising discounted facelifts under the name “Dr. Stein Aesthetic.”


During our visit, she pulled up the page. The account appeared to be connected to a clinic in Shanghai, but we couldn’t find a clear clinic name, surgeon credentials, or verifiable location. The page featured dramatic before-and-after photos of patients immediately after “2-hour facelifts,” all at deeply discounted prices.


The images did not look authentic. There was no transparency about who was performing the procedures, where they were being done, or what qualifications the surgeon held. Lucille was understandably confused.


Even though I do not specialize in facelifts, we spent our consultation discussing something far more important than any single procedure — how to verify medical information found on social media and how to safely choose a surgeon.


Lucille also shared that she previously had a parotid tumor removed from her right cheek, which resulted in facial nerve weakness for six months. That history makes facial surgery more complex and higher risk. In her case, careful surgical planning and expert nerve preservation would be critical.


Rather than encourage a quick solution, I gave her the name of an excellent, board-certified facial plastic surgeon in our area. Given her medical history, she should be especially cautious when selecting someone to operate near her facial nerves.


That conversation reinforced something I see more often:


Social media can make surgery look fast, simple, and discounted. Real surgery is none of those things.


Choosing Safety, Quality, and Long-Term Results Over Short-Lived Looks


Each year brings new buzzwords in aesthetic medicine — procedures that go viral on social media, celebrity transformations that spark “must-have” lists, and quick-fix recommendations that seem too good to be true.


While innovation drives our field forward, not every trend is safe, effective, or right for your body and goals. Trends can oversimplify complex decisions, and chasing them without thoughtful guidance can lead to disappointment — or worse, avoidable complications.

Trend Spotlights: When Popular Doesn’t Mean Proper


Barbie Waist & Rib Remodeling Procedures


“Barbie waist” or rib remodeling surgery is another trend gaining attention online. These are extreme cosmetic operations that narrow the lower rib cage to create a highly accentuated waistline. While it might appear appealing in a trendy photo, the reality is that removing or altering ribs carries significant risks — including chronic pain, breathing issues, and long-term structural changes. Such procedures should only be considered after an in-depth consultation with a board-certified plastic surgeon who can assess your anatomy, discuss realistic outcomes, and clearly explain risks.


Brazilian Butt Lifts Without Ultrasound Guidance


One of the most talked-about procedures is the Brazilian Butt Lift (BBL), but when done improperly it can be dangerous — even life-threatening. Surgeons now understand that injecting fat too deeply, especially into or beneath the gluteal muscles, raises the risk of fat entering the bloodstream and causing a fat embolism. Fat embolisms can block the lungs or heart and have been linked to deaths in poorly performed BBLs. Modern safety standards recommend using ultrasound during a BBL to ensure fat is placed in the right tissue plane, but that is not always practiced in unregulated settings. Choosing a board-certified plastic surgeon and an accredited facility is essential for safely implementing these techniques.


“Palm Line Surgery” and Other Novel Trends


Some trends being discussed online — like surgical alteration of palm lines to “change your fate” — don’t have scientific grounding in plastic surgery or long-term safety data. These procedures stem from cultural beliefs rather than evidence-based practice, and they highlight how aesthetic surgery can be co-opted by trends that sound appealing but lack real value or predictable outcomes.


What Should Drive Your Decisions?


Instead of asking, “What’s trending?”, a better question is, “What’s appropriate for me?” Aesthetic decisions should be:


✔ Based on your unique anatomy

✔ Grounded in safety and evidence

✔ Focused on long-term outcomes

✔ Guided by a qualified surgeon


That’s why trends should always be weighed against clinical judgment, safety considerations, and your personal goals.

A board-certified plastic surgeon has completed rigorous training, passed comprehensive exams, and demonstrates a deep understanding of anatomy, surgical techniques, and complication management. Procedures — whether major or minor — should be performed in accredited facilities where:

Choosing convenience or lower cost over an accredited environment can compromise safety — and it’s a risk that isn’t worth taking.
